Erker Clinic- Privacy Policy 

Erker Clinic, LLC does not sell, rent, or share your personal information, phone number, or SMS consent data with third parties for marketing or promotional purposes. Messaging data is used only for patient communication and clinic operations. This Notice of Privacy Practices describes how your medical information may be used and disclosed, and how you can access this information. Please review it carefully. We are committed to protecting the privacy and security of your protected health information (PHI). 1. Our Legal Duty We are required by law to maintain the privacy of your health information, provide you with this notice of our legal duties and privacy practices, and notify you if a breach occurs that may compromise the privacy or security of your PHI. 2. How We May Use and Disclose Information We may use and disclose your PHI for purposes of treatment, payment, and healthcare operations, including: • Treatment – coordinating or managing your healthcare with other providers. • Payment – billing-related activities such as processing membership fees or services not covered under membership. • Healthcare Operations – quality assessment, staff training, accreditation, and business management. Other disclosures permitted or required by law include public health reporting, health oversight, judicial proceedings, law enforcement, organ donation, research (with safeguards), serious health or safety threats, and specialized government functions. 3. Your Rights As our patient, you have the right to: • Request restrictions on how we use or disclose your PHI (though we may not be required to agree). • Request confidential communications (e.g., to receive information at an alternate address). • Inspect and obtain a copy of your PHI (fees may apply). • Request amendments to your PHI if you believe it is inaccurate. • Receive an accounting of disclosures made outside treatment, payment, and operations. • Receive a paper copy of this Notice at any time. 4. Communications Policy We communicate with patients primarily through our office phone lines and designated contact tree, which is provided to you separately. Texting may be used for scheduling but is not intended for medical emergencies. Urgent or emergent needs must be communicated by direct phone call to the provider. Email may be used only for billing-related communications at billing@erkerclinic.com. Our patient portal is used solely for billing purposes at: https://erkerclinic.atlas.md/hub/login. 5. Changes to This Notice We reserve the right to change the terms of this Notice and to make the new provisions effective for all PHI we maintain. If changes are made, the updated Notice will be available in our office and on our website. 6. Complaints If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
